
Claire McKenzie-Sime
Since qualifying in 2018, Claire has gained experiences working in adult forensic in-patient, adult community mental health and community rehab for older adult settings for people with complex care needs. She has experience and knowledge of the journey to transition people from one setting to another, including discharge planning from hospital to the community and has had numerous opportunities to extend her knowledge in the use of specialist OT assessment tools such as Barthel, MOHO and COPM.
Claire is skilled at assessing physical health issues to prescribe assistive equipment, minor environmental adaptation and maximisation of patient performance/independence. She has been able to explore trauma informed practice and has developed her skills using Vocational Rehabilitation tools such as WRAP to provide an education/employment pathway whilst working closely with IPS and WDS. Overall, Claire is committed to constantly extending her knowledge and experience to provide the best possible level of care to clients, working towards achievable, long-term goals.
